Medical Information Release

After referring an athlete to Orthopaedic Specialists of North Carolina, a coach, athletic trainer or school/team representative may require or request information regarding the visit. Often this information is essential to the team for practice and game preparation and planning and more importantly is essential to keeping the athlete safe and allowing appropriate re-entry to sport. Frequently the team physician will call the coach, athletic trainer or school/team representative to discuss the findings. Any medical information given to a coach, athletic trainer or school/team representative in spoken or written form is confidential.
