How to Refer a Patient
There are two ways to refer a patient.
REFERRALS MADE BY FAX
Fax the patient demographics, pertinent medical records, test results, and insurance information to (919) 562-9425.

We have four OrthoNC Injury Express Urgent Care locations to serve patients and are proud to offer rapid, orthopedic care at our Wakefield-Wake Forest, North Hills, Wendell Falls, and Henderson locations.
